I have the same issue. 2011 Honda pilot, just over 290k miles. Oil changes every 2-3 months, last oil change, was empty on oil. I have it leaving from 2 spots one is drain plug, don't know where other spot is.

I can feal the vibration in my engine during trafic signal stop, and it will disappear after putting gear to neutral,
And sometimes. Vibration can be felt during shifting gear while speed control on.

Honda Pilot 2008 V6 FWD 165000 miles
This only happens when the engine been running for at least 20 minutes or more. When driving at highway speeds and needing to accelerate to pass someone or change lanes it will start to hesitate at around 3500 rpm when trying to downshift. Its is similar to how it would "bounce" off the rev limiter except its obvious not at the high rpms. Also, sometimes when slowing down for a red light and then the light turns green just before I come to a complete stop and start to accelerate again it will hesitate for a couple of seconds.

Checked transmission fluid, it's clean and full. Has a P2646 code as well as a code for an O2 sensor that I'm not stressing about.
Yes same. But my Pilot seems to hesitate, then hard click into gear when I accelerate after slowing down for a light.
P2646 indicates that your variable valve timing isn't working. The computer isn't seeing a signal from the pressure sensor when it expects to see pressure. It can be as simple as low engine oil or as bad as a complete engine flush, replacement of the vvt oil filter housing and solenoid. If you keep oil changed frequently it could be a bad pressure sensor but usually it is related to poor maintenance. There are 3 screens in the system that will need cleaned as well if it due to poor maintenance. The aftermarket parts don't usually work well as far as the solenoid and housing so should be genuine honda when replace.
I keep up with maintenance, we attempted to clean the screens but it didnt solve the issue. Ended up having to replace the rocker control arm actuator. We did get an aftermarket part, its been over a month and still doing fine.

Replaced rocker control arm actuator. Its right on top of the engine. Took about 15 minutes to fix. Part was aboht $250 dollars. I just procrastinated on it.

Honda Pilot (and other models of larger Honda vehicles) have Variable Cylinder Management (VCM) that shuts off 1 to 3 cylinders if the power is not needed while cruising. This leaves the valves closed so that as they go up and down, causing vacuum in the cylinder and the oil to get into the rings and the vehicle will use oil and many other problems.

When I start the engine the front a/c blow cold, after a few minutes could be 10 minutes or 45 minutes blow warm or hot air.

Check A/C gas and try refilling it and check there if any leaks are visible

My car turns on but when accelerating over 2000 it turns off. The radio stays on but I can’t drive. I’ve already replaced the battery, No engine light is on and I’m wondering if this could be related to a fuel pump issue or a crank shaft sensor… that’s what the mechanic said but anyone else have ideas? Suggestions?
