Limp mode when accelerating

(2016 Ford Mondeo TD Titanium. ) When I accelerate joining the Freeway to reach 100kms/hr, the spanner pop up and goes into limp mode. I need to pull up on the emergency strip. Turn off the engine. And it goes away. The spanner icon used to light up with any increase of speed whilst driving in the 80km/hr to 100km/hr speed window. But now only when I'm accelerating from 0 - 100km/hr with a hard acceleration. It doesn't do this if I go up in speed at a gentle, slower climb. I had it looked at and the mechanic found a hole in the turbo hose. He said that that was thd cause. So that was replaced but like I said. It's still happening. Any suggestions on why or how to fix this would be very helpful and appreciated. Thanks
